Which farm is this happening on and which tools and size of tool are you using when this happens? Does it still happen if you use a different size tool? When did this start happening?

What have you already tried to try and resolve this yourself? For example, making sure any updates are done on your computer, checking browser is up to date, clearing caches/history on the browser, seeing if you get the same problem using a different browser, running memcache.
